Decision No. 5772/QD-BYT dated December 20th, 2021 of the Ministry of Health on sample and procedures for issuance of “vaccine passport”

Under this Decision, “vaccine passport” shall be issued in form of QR code, consisting of these contents: full name; date of birth; nationality; targeted disease; number of dose; date of vaccination; vaccine, etc.

A QR code expires after 12 months from the date on which it is generated.

This QR code is granted by vaccination centers under the procedures applied consistently nationwide.

This Decision comes into effect from the day of signing.
