This Decision promulgates the process for extending household-based health insurance cards on the Public Service Portal or the Public Service Portal of Social Insurance with reduction in health insurance premiums.

Accordingly, in case requiring extension of household-based health insurance cards with reduction in health insurance premiums, individuals will choose to declare on the National Service Portal or on the Public Service Portal of Social Insurance and make a full declaration of information fields, including:

a) Information of the Householder: Full name; date of birth; sex; personal identification number;

b) List of members with the same permanent residence registration or the same temporary residence registration who request for extending their household-based health insurance cards: health insurance card number, full name, date of birth, sex, relationship with the householder, number of months that each member requests for extension

After the individuals declare full information, the software system will automatically inspect, compare and verify the information about residence address/temporary address; health insurance participation status of each member and determine the exact payable premiums of each household member in descending order of household-based health insurance premiums.

The declarants will receive the notice of payable premiums (with the detail premium of each health insurance card number) and make the online payment of payable premiums.
